Start by creating a new file of header size. Apply a nice gradient as shown.

Insert an image of a building cutout.

Duplicate layer. Enable lock transparent pixels from layer palette. Press Ctrl+T. Right click the layer and select distort. Fill it with gray color. Adjust nodes to create a perspective shadow.

Create a white rectangle and rotate as shown.

Ctrl+Click the layer thumbnail to get the selection. Select the building image layer and press Ctrl+J to copy paste the selection into a new layer. Press Ctrl+I to invert the selection. Move this layer above all the layers in the layer palette.

Double click the layer to open the layer style window. Apply settings as shown.
