Start creating a new file of header size. Create a rectangle using rectangular shape tool. Place it on the bottom.

Double click the layer to open layer style window. Apply settings as shown.

Create another rectangle using rectangular shape tool. Place it on the bottom.

Double click the layer to open layer style window. Apply settings as shown.

Duplicate both the layers. Press Ctrl+T. Right click and select Flip Vertical and place on the other side.

Create a rectangle using rectangular shape tool.

Double click the layer to open layer style window. Apply settings as shown.

Create "Square frame" shape using custom shape tool.

Double click the layer to open layer style window. Apply settings as shown.

Create a small rounded rectangle using rounded rectangle shape tool. Apply same layer style. Create a circle using elliptical shape tool. Apply some bevel&emboss layer style.

Duplicate both the layers and place on each corner of the header.

Select center big rectangle and frame layer. Duplicate, scale down and merge both the layers. This will create a button. Duplicate to create more buttons.

Select and merge all the layers. Duplicate the layer and change the layer mode to "Overlay".
