Start by creating a shape as shown using pen tool.

Duplicate the layer, change the color and move to right.

Duplicate both the layer. Press Ctrl+T. Right click and select Flip Vertical and place on the other side.

Create another duplicate of these layers and place as shown below.

Create a rectangle using rectangular shape tool. Place it on the top.

Change the layer mode to "Soft Light".

Duplicate the layer, move a few pixels upwards. Change the layer mode to "Normal" and reduce the opacity to 50%.

Duplicate both the layers. Press Ctrl+T. Right click and select Flip Vertical and place on the other side.

Populate the header with links or any other information relevant to your design.
